Request to Adapt or Reproduce Materials of KFL&A Public Health

Permission to adapt and or reproduce the Work of KFL&A Public Health (referred herein as the Work) is subject to the following conditions, if approved:

1. The User acknowledges the Work and copyright is owned by KFL&A Public Health. 
2. All copies of the Work shall bear the following notice:  “Reprinted (or adapted) with the permission of KFL&A Public Health.”   
3. If the KFL&A Public Health logo is used, it must be printed in teal, white, or black.
4. The User shall use the Work for only the requested purposes.
5. The Work shall not be used for profit.
6. The User shall not allow any other person, agency, organization, or other entity to photocopy or otherwise reproduce in any manner the work or any portion thereof without the prior written consent of KFL&A Public Health.
7. All stock images, audio files, and video clips included in the Work are purchased and licensed by KFL&A Public Health. The requestor is responsible for purchasing and licensing the aforementiond items from the original source(s) before adaptation. Reproduction of purchased and licensed items without permission is prohibited. 
8. If the User fails to comply with any of the above-noted terms and conditions, it shall immediately, upon the request of KFL&A Public Health, return to KFL&A Public Health all copies of the Work and discontinue all use thereof.
