Notice of Intent to Operate a Personal Service Setting

Effective July 1, 2018, all operators intending to operate a personal service setting in Ontario must notify the local Public Health Unit of their intent, as per subsection 3(1) of the Personal Service Settings Regulation 136/18 made under the Health Protection and Promotion Act R.S.O. 1990 Ch.7. 

Please be advised, the completed form must be submitted to KFL&A Public Health at least 14 days before commencing operation.
