Notice of Intent to Operate a Personal Service Setting

Effective July 1, 2018, all operators intending to operate a personal service setting in Ontario must notify the local Public Health Unit of their intent, as per subsection 3(1) of the Personal Service Settings Regulation 136/18 made under the Health Protection and Promotion Act R.S.O. 1990 Ch.7. 

Please be advised, the completed form must be submitted to KFL&A Public Health at least 14 days before commencing operation.

Floor plans

Floor plans must include the locations and specifications for:

• hand washing sinks and supplies,
• washrooms,
• reprocessing space and sinks,
• finishing materials for floors, walls, and ceilings, and
• service rooms.

The personal information on this form is collected under the authority of the Health Protection and Promotion Act and will be used for the administration of Public Health programs.
