Influenza Vaccine Order Form

Initial and priority-based influenza vaccine orders have been completed. You many submit additional orders based on your current supply and clinic needs. We will continue to process influenza vaccine orders based on supply and we will work with vaccine providers to increase allocations as needed.

Order and pick-up instructions

Napanee based orders

• Napanee orders need to be submitted by noon on Wednesday to be ready for pick-up at the Napanee office the following week. 
• Napanee office pick-up available Monday 1:00 to 4:30 pm and Tuesday to Friday 8:30 am to 12:00 pm and 1:00pm to 4:30pm.

Kingston based orders

• Orders will be ready three business days after the order is placed. Kingston office pick up available Monday to Friday 8:30am to 4:30pm.

You may place your order now. All orders will be processeed as soon as product is available.

Please note that in some cases, processing may take longer than 3 business days due to the approval process, ministry- designated ordering timelines or potential shipping delays. If your order is urgent, please call us directly at 613-549-1232 after submitting your order. We will do our best to accommodate your request.

Vaccine ordering

• Fluzone® Trivalent Inactivated Vaccine (TIV) and Fluzone® High-Dose Trivalent Inactivated Vaccine are different products. Fluzone® High-Dose Trivalent Inactivated Vaccine is only authorized for those 65 years of age and older. Please use caution when administering Fluzone® products to ensure that the right vaccine is being administered to the right person.

Trivalent Inactivated Vaccine (TIV)

Eligibility: 6 months and older

High-Dose Trivalent Inactivated Vaccine (TIV-HD)/ Adjuvanted Trivalent Inactivated Vaccine (TIV-Adj)

Eligibility: 65 years and older

There is no preferential recommendation for the use of TIV-HD versus TIV-adj vaccine for this age group

Please provide dates of influenza clinics planned at your facility if applicable, and the approximate number of required doses

Delivery

As per the Vaccine Storage and Handling guidelines vaccine transport is only permitted in a hard-shell cooler, cooled to between 2 to 8 degrees and monitored with a digital maximum and minimum thermometer capable of measuring in 0.1 degree increments.
